Once you receive a confirmation email from this form, please reply with copies of photo identification and proof of address. Satisfactory photo identification includes either:

- Passport or
- Photo of front and back of your drivers' licence as well as a bank card or other card with name embossed.

Satisfactory proof of physical address includes any utility bill, bank statement or rates demand addressed to you and sent to your home address.
